I think what you're talking about is very deep theta (mind awake/body aleep). On Dale Power's website, he writes that in theta you lose your track of thoughts and can completely forget what you wanted to do in theta. This has been my experience too - I start doing visualization or affirmations, and then a few seconds later I realize that my mind is empty and I try hard to remember what was I trying to visualize. And I also lose consciousness and then come back several times during meditation.
